Beyond Telehealth and AI-Scribing: Addressing the Limitations of Live Care

Telehealth and the limitations of live care

While telehealth was once hailed as the ultimate digital transformation of healthcare, it often merely replicates the inefficiencies of the traditional clinic in a virtual space.  Because telehealth remains a synchronous model, it requires both the patient and the clinician to be digitally present at the exact same moment.  This creates a high-pressure bottleneck where information gathering is restricted to a single, brief window of time.  In contrast, asynchronous care allows for a more longitudinal and high-fidelity approach to data collection.  Research suggests that asynchronous communication can lead to more thoughtful, detailed patient disclosures and improved clinical efficiency, as patients are not rushed by the constraints of a 15-minute live connection (JMIR 2021).

Is ambient scribing capturing a complete clinical picture?

The current excitement surrounding AI ambient scribing – software that automatically generates medical notes from live clinical conversations – also fails to address this fundamental structural flaw.  While AI tools can significantly reduce the documentation burden for providers, they are still tethered to the live encounter.  If a symptom or patient concern isn’t voiced during that specific synchronous visit, the AI cannot capture it.  This limits the clinical narrative to a snapshot rather than a complete picture.   Despite being promoted to reduce administrative workloads, AI ambient documentation software did not improve overall outpatient clinician efficiency, signaling a need for alternative implementation strategies and further research (NEJM 2024).   Asynchronous delivery breaks this cycle by enabling the gathering of richer, more complete symptom data that is entirely unlinked from a live meeting.  This method captures patient insights in their natural environment, providing a level of clinical nuance that even the most advanced AI scribe cannot extract from a hurried live session.

Addressing the clinical information gap

MELD Health’s solutions are designed for a new era of medicine that prioritizes data depth over calendar availability.  By enabling richer symptom information to be captured outside the clinic, MELD empowers both the patient and the physician with a comprehensive view of health that exists between traditional appointments.  This model not only facilitates more informed diagnostic pathways but also dramatically expands patient access.  By moving care delivery into a remote, asynchronous framework, we remove the geographic and scheduling barriers that have long hindered the patient experience, ensuring that quality care is a continuous process rather than a sporadic event (SageJournals 2023).
